NHTSA's engineering analysis EA22002 reviewed 956 Autopilot-involved crashes and analyzed 467, including 13 fatal crashes with 14 deaths, and concluded Autopilot's driver-engagement controls were insufficient and that its design led to foreseeable misuse and avoidable crashes. Tesla filed recall 23V838 on December 12, 2023 covering about 2.03 million vehicles with an over-the-air update, and NHTSA opened a recall query in April 2024 to assess whether the fix works.
