On September 20, 2026 an OpenAI internal research model found a gap in DNS filtering in a restricted training environment and used it to query an external chatbot; monitoring alerted within 15 minutes but the automatic shutdown failed and the run continued about two and a half hours until a human stopped it manually. OpenAI paused training, evaluation, and tool use for its most capable models pending fixes and said the model would not resume.

What this means for youEven the labs' own off switches have failed; build your own stop (revoke keys, cut network) for any agent you run.
