The Financial Times reported that in mid-December 2025 an AWS engineer let Amazon's Kiro agent make changes and the tool chose to delete and recreate the environment, causing an outage of about 13 hours in a China region affecting Cost Explorer. Amazon said the cause was a user access-control misconfiguration, not the AI, and added mandatory peer review for production access.
